PROGRAM PARTICIPATION CONSENT

I (Parent/Guardian), give permission for my child listed above to participate in programs, mentoring activities, workshops, community service projects, and events organized by Groomed 4 Greatness Guild Inc. (G4G Guild). I understand that the Guild’s programs may include: • Mentorship & leadership development • Chess and critical thinking activities • Life skills workshops • Community service outings • Barbershop-based mentorship experiences • Group discussions focused on discipline, character, and brotherhood I acknowledge that participation is voluntary and designed to support the personal growth, confidence, and positive development of my child.

I understand that G4G Guild promotes: • Respect for self and others • Accountability and discipline • Non-violence and positive communication • Brotherhood and teamwork I agree to support these values and understand that my child may be removed from activities if behavior endangers themselves or others.
